What 12 labels per A4 sheet means
A count of 12 per sheet does not fix the label size: the 9 sheets here use 6 sizes and 3 grids (3 × 4, 2 × 6 and 4 × 3). Labels cover between 70 % and 97 % of the A4 page; the rest is margin and gaps.
Always choose the template by the code printed on the box: two 12-up sheets with different sizes or margins need different templates, even though the count matches. The most common size at this count is 63.5 × 72 mm (3 sheets).

Which 12-up sheet to pick
Avery J8164 is the default here because Avery codes are the usual reference for compatible sheets, and it shares its exact layout with 1 other sheet on this list, so its template prints on those too. Its labels are 63.5 mm (2.5″) by 72 mm (2.833″).
If you need bigger labels at the same count, the largest are 105 × 48 mm on Herma 4457; the smallest are 78.7 × 46.5 mm on Herma 4828.
